fuel was only tested on ground, is the only thing I heard so far. Sad times for space travel, and the man who died. but I'm sure we will learn a lot from this and hopefully will not happen again.

Seems that the re-entry device deployed early, there has been slight confliction between this report and another I read which seemed to suggest that the device was unlocked by the pilots, however this report states it deployed without command.

Early days before we find out what happened though

It appears that the co-pilot unlocked the feathering system early in the ascent and the system then deployed without the pilots input.
